After-Care Re-Entry Program
The target population for the Latin American Youth Center After-Care Re-Entry Program are court appointed youth, ages 14 to 21 primarily from wards 1,2, and 4. The majority of these youth will be gang involved or affiliated Latino or other immigrant and minority youth. The program serves both males and females. The program seeks to improve our ability to develop pre-release aftercare plans by working in close collaboration with judges, probation officers, YSA, and to serve as a liaison between youth and these entities to ensure a more seamless delivery of services.
The LAYC After-Care Re-Entry Program approach is focused on young people’s strengths rather than their weaknesses or problems. Our intervention is aimed at strengthening four areas of self-development: competence, usefulness, belonging and power. The LAYC recognizes the importance of providing positive developmental opportunities in a time of change and recovery.

Goals
- Significantly reduce the rate of re-arrest and/or re-incarceration among participating youth
- Decrease gang involvement and activity
- Increase participants, life skills, educational attainment and job skills
- Strengthen participants’ abilities to live productive lives free of the juvenile justice system and gangs;
- Assist participants in acquiring organizational leadership skills, strengthening their values, creating a sense of community and promoting cross cultural understanding among youth so they can create productive futures in their lives and the community
Objectives
The objective of the After-Care Re-Entry Program is to provide 15 adjudicated youth with culturally appropriate, bilingual after care/reentry services, with the following outcomes:
- 100% of participants will develop and implement an educational or vocational plan (60% will have an educational plan, 40% will have a vocational plan;
- 60 to 70% of participants will not return to the juvenile justice system and will remain free of gang involvement;
- 100% will attend workshops on gang prevention/intervention, community service, job readiness and life skills, violence prevention; etc.;
- 100% will be assisted into stable living conditions; and
- 40% will be referred for other services (within the LAYC or to other service providers) i.e. mental health or substance abuse counseling, legal services, etc.
The LAYC After-Care Re-Entry Program will provide adjudicated youth who are being released or on probation with aftercare and reentry services including:
- Psychosocial Assessment
- Individualized Case Planning
- Case management
- Assistance with attaining employment or enrolling in educational programs
- Tutoring/Mentoring
- Activities emphasizing violence prevention, gang prevention/intervention, health, life skills, and job skills.
- Referrals to individual, group, family, and substance abuse counseling and mental health services
- Working in conjunction with YSA agents, Probation Officers, Social Workers, group homes
